Peter D. Calder, M.D.

Dr. Calder joined the team of Berks Eye Physicians & Surgeons in October 1993. Born in Reading and raised in Central Pennsylvania, Dr. Calder attended Penn State, where he graduated Summa Cum Laude. Upon receiving his medical degree from Pennsylvania State College of Medicine at the Milton S. Hershey Medical Center, Dr. Calder completed his residency at the University of Rochester Medical Center, New York.

In addition to receiving Phi Beta Kappa, Alpha Omega Alpha (National Medical Honor Society) and other academic honors, Dr. Calder is a Fellow of the American Academy of Ophthalmology and a member of the American Medical Association, the Berks County Medical Society, and the Pennsylvania Medical Society. He was selected as an Outstanding Young American in 1998.

Prior to joining Berks Eye Physicians & Surgeons, Ltd., Dr. Calder served in the United States Navy as the Attending Ophthalmologist at the Great Lakes Naval Hospital, in Great Lakes, Illinois. He is currently on staff at the Reading Hospital and Medical Center, and the Berkshire Eye Surgery Center. He specializes in the advanced "no-stitch, no-patch" cataract surgery and medical glaucoma treatment.

Dr. Calder, his wife Sharon (a nurse practitioner in West Reading) and their son all enjoy traveling. They reside in Mohnton.

Domenic C. Izzo, Jr., M.D.

Dr. Izzo is a board-certified ophthalmologist specializing in vision correction surgery, including advanced LASIK, as well as cataract and intraocular lens implant (IOL) surgery. As a member of the acclaimed Berks Eye Physician team, Dr. Izzo uses the highest quality LASIK surgical techniques and technologies to help patients eliminate their dependence on glasses and contact lenses. Dr. Izzo is also a leading cataract surgeon, having performed 15,000 cataract and intraocular lens implant procedures, and was one of the first ophthalmologists in Berks County to perform small incision, no-stitch cataract surgery. He has expertise in presbyopia-correcting lens implants (ReStor, ReZoom, and Crystalens) designed to eliminate the need for reading glasses and bifocals, as well as Toric IOLs, which eliminate astigmatism.

Dr. Izzo completed a Bachelor of Science degree with magna cum laude honors at Muhlenberg College and a medical degree at Hahnemann University. He served an internship at The Reading Hospital & Medical Center, and an ophthalmology residency at Hahnemann University Hospital. Dr. Izzo is a member of a number of prestigious medical organizations including the American Medical Association, the American Society of Cataract and Refractive Surgery, the Pennsylvania Medical Society, the Pennsylvania Academy of Ophthalmology and the Berks County Medical Society. In addition, he is a Fellow of the American Academy of Ophthalmology. He serves as Medical Director for the Berkshire Eye Surgery Center.

Francisco L. Tellez, M.D., FACS

Dr. Tellez is a board-certified ophthalmologist with over 20 years of experience. He joined the Berks Eye Physicians & Surgeons team in 1997. Dr. Tellez specializes in small incision, no-stitch, no patch cataract and intraocular lens implant (IOL) surgery. He uses the latest technologies, such as presbyopia-correcting implants to eliminate the need for reading glasses and bifocals, as well as toric implants to correct astigmatism.

Dr. Tellez is a fellowship-trained glaucoma specialist with expertise in all aspects of medical and surgical glaucoma diagnosis and management. His individualized care plans offer the full range of treatment options, including the newest medications, the latest laser technologies, and the most effective surgical procedures to protect your vision and enhance your quality of life. Dr. Tellez also has a special interest in the treatment of cataracts in the glaucoma patient.

A Phi Beta Kappa graduate of Cornell University, Dr. Tellez received his medical degree from the New York University School of Medicine in New York City. He served his internship in internal medicine at the Johns Hopkins Bayview Medical Center. He completed his residency in ophthalmology at the Long Island Jewish Medical Center of the Albert Einstein College of Medicine in New Hyde Park, New York, where he served as chief resident. He completed additional specialty fellowship training in glaucoma at the Emory Eye Center in Atlanta, Georgia, which is consistently ranked among the best ophthalmology training programs by U.S. News & World Report.

Dr. Tellez is certified by the American Board of Ophthalmology. He is a Fellow of the American College of Surgeons and a Fellow of the American Academy of Ophthalmology. He is a member of the American Society of Cataract and Refractive Surgery, American Medical Association, Pennsylvania Academy of Ophthalmology, Pennsylvania Medical Society, and Berks County Medical Society.

Dr. Tellez is bilingual and speaks fluent Spanish. He plays the saxophone with the Praise Teams of the West Lawn United Methodist Church.

Jacquelyn M. Horst, O.D.

Jacquelyn M. Horst, O.D. provides primary eye care to patients of all ages, specializing in eyeglasses and contact lenses. Dr. Horst has experience with advanced contact lens fits; including multi-focal contact lenses, rigid gas permeable lenses, corneal refractive therapy, and hybrid contact lenses. She is also Nationally Board Certified to diagnose and treat ocular diseases. Dr. Horst is a member of the American Optometric Association, Pennsylvania Optometric Association, and she currently stands as secretary of the Berks County Optometric Society.

Dr. Horst is a native of the Reading area and graduated from Governor Mifflin High School. She received her Bachelor of Science in Biology degree with cum laude honors from Millersville University in Lancaster, PA. She then went on to receive her Bachelor of Science in Visual Science and Doctor of Optometry from Pennsylvania College of Optometry at Salus University in Philadelphia through their accelerated baccalaureate O.D. degree program.

Dr. Horst enjoys cooking, tennis, traveling, and spending time with her husband and two dogs.
